How to Download and Submit Your Long Form Application

We’ve made it simple for you to download and complete your long form application application. Follow the steps below to ensure your application is submitted successfully:

  1. Download the Application
    Click here to download the application form.

  2. Complete the Application
    Open the downloaded file and fill out all the required fields. Ensure that all information is accurate and your case matters and CLE is within 3 years before your short form expiration date. Please visit your profile to view your short form expiration date. 

  3. Save Your Completed Application
    Once you have completed the form, save it to your computer.

  4. Submit Your Application
    Email your completed application to Lisa Aukward at [email protected]. Please use the subject line: "Long Form Application Submission: [Your Full Name]."

Important Notes:

  • Ensure all required documents are attached to your email before submission.

  • Applications must be submitted by the deadline (short form expiration date) to be considered.
